Application Forms
Holistic Passings Foundation exists to ensure that every hospice patient can access holistic services that soothe the body, calm the mind, preserve dignity, and uplift the spirit. We do not give money to patients or hospices. Instead, we coordinate and pay for approved holistic services directly, working with the hospice’s Holistic Services Coordinator or Volunteer Coordinator. Support may also extend to the primary caregiver.

HPF coordinates and pays providers directly; no funds are disbursed to the hospice or patient.

Final service types, providers, and number of sessions per month are at HPF’s discretion based on needs, appropriateness, availability, and funding.

Approved hospices must submit monthly electronic reports of services delivered and outcomes.
